Do you need upper or lower eyelid surgery?

The aging process often takes a significant toll on the area around the eyes. The delicate skin of this area often begins to droop or sag. This can lead people to look permanently tired, or even angry or sad. It also contributes to making people look older. Many people also find themselves with bags under the eyes as they age. This occurs as the tissues that support the undereye fat pads droop over time, so the fat pads migrate downward and become more visible. 

For those who have concerns about the appearance of their eyes, a surgical procedure known as blepharoplasty may be able to help. There are actually two different types of blepharoplasty – upper and lower. Although they both involve the eyelids, they’re actually very different procedures, with different goals.

What are the differences? How do you know which one is right for you?

Upper blepharoplasty addresses sagging eyelids

With upper blepharoplasty, the primary goal is to address sagging eyelids. A/Prof Marucci makes the incision along the natural crease of the upper eyelid. He will then remove excess drooping skin and fat, and tighten the skin of the eyelid. It’s important that this be done with excellent surgical technique, because removing too much tissue can lead to a permanently “surprised” look or even to trouble closing the eyes. Once the right amount of tissue has been removed, he carefully closes the incisions. After healing, the resulting scars are very discreet.

Lower blepharoplasty addresses eye bags

With the lower eyelids, there’s generally a different focus to the surgery. It’s targeted at the tissues just underneath the eyes, to address issues like bags under the eyes. After making the incision at the lash line or just inside of the eyelid, A/Prof Marucci will reposition the fat pads in the area. In some cases, he may remove some tissue, but it’s very important not to remove too much or the patient will look “hollowed out” rather than rejuvenated. If there’s also excess skin on the lower eyelid, he’ll remove this and tighten the remaining tissue, before closing the incisions. The resulting scars are well-hidden at the lash line or inside of the eyelid.

Which one is right for you?

If your primary concern is sagging or drooping eyelids, then upper blepharoplasty may be right for you. If you’re mainly bothered by bags under your eyes, then lower blepharoplasty may be helpful. If you have both of these concerns, it’s possible to have both types of eyelid surgery in a single combined procedure. 

In some cases, adding a brow lift will help to enhance the results of upper blepharoplasty and get the results that you’re looking for. This can also be done at the same time, so that you only have to plan for a single surgery and recovery period.
